  • Responsibilities

    Reporting to the Director of Contracts, the Ability One Case Manager is responsible for Agency’s compliance with Federal Ability One regulations as they relate to supported employees under the Ability One contracts.

     

    Essential Functions

    Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

    • Day to day compliance and case management of the Ability One contracted programs.
    • Conduct formal orientation and all other intake procedures for new supported employees with disabilities within the Ability One program.
    • Complete all applicable evaluation, eligibility, interviewing, orientation, badging and ongoing assessment procedures for employees with disabilities to ensure program eligibility requirements are maintained. Implement short-term workplace accommodations; as necessary
    • Provide benefits planning services to individuals receiving social security benefits (hours, accommodations, wage verification, dispute proposed actions, complete all SSA paperwork.)
    • Develop, implement, observe and modify an Individual Employment Plan (IEP) and Annual Competitive Employment Evaluations for each disabled employee in the Ability One per program requirements.
    • Cross-train Project Manager, Supervisors, and Team Leads on case management procedures, documentation requirements, providing additional supports to disabled employees, documentation, and standards development.
    • Communicate all applicable information for each disabled employee and work closely with the work supervisor and the employee to ensure mutual understanding of goals and objectives, maintain employee morale and good working relationships between the supervisor, co-workers and management staff.
    • Participate in job related client training sessions, as needed.
    • Assess each disabled employee's potential to benefit from external service referrals and communicate authorized information to external agencies to facilitate them.
    • Complete employment exit paperwork needed in the case files, within the established time frame.
    • Maintain all Ability One case management files according to the established policies, procedures and time frames.
    • Establish follow-up contacts/reports on at least 30, 60 and 90 day basis for all exited direct labor employees who moved to competitive employment.
    • Collect and submit to Data Manager, new hire, placement and termination information/statistics in a timely manner.
    • Participate in compliance audits as needed.
    • Participate on Agency committees and community organizations as requested.
    • All other duties assigned by the Director of Contracts

     

     

    Qualifications

    The Ability One Compliance and Case Manager will be a proven leader with the following:

    • Prefer a Bachelor's degree in rehabilitation, psychology or related field, and 2 years of experience.
    • Microsoft Office and Windows proficiency
    • Demonstrated multi-tasking experience.
    • Must possess excellent employee relations and customer service skills
    • A high degree of sensitivity towards all persons with or without disabilities/disadvantages.
    • Must have reliable transportation, clean MVR, valid license and insurance to transport consumers and/or drive for company-related business.
    • Understanding of disability service issues as they pertain to Case Management.
