The Life Coach is responsible for the delivery of case management as part of WRAP Services to job seekers, participants, and supported employees who are enrolled in various Mission Services programs. The Life Coach will follow alongside each participant to refer to various Mission Services employment programs from the start of their enrollment to the end. The Life Coach may also refer participants to receive assistance in benefits planning, financial literacy, and/or to refer to other community organizations.


Essential Functions

  • Conduct intake orientation for new participants enrolled in WRAP Services
  • Complete all evaluations and assessments applicable to their program enrollment
  • Provide case management and connect to benefits planning services for all employees
  • Develop and implement an Individual Support Plan (ISP) for each participant
  • Review and modify ISPs for each participant on a six-month basis
  • Provide job related individual and group training sessions, as needed
  • Provide for supportive services during emergency situations for all participants enrolled in WRAP Services
  • Schedule and conduct formal staffing for the development and review of ISPs and informal staffing to resolve any unusual circumstances that may arise
  • Assess, on a regular basis, each participants potential to benefit from an external service referral
  • Provide a service referral for any participant who demonstrates the potential to benefit
  • Assume responsibility for communicating authorized information to any external agencies that will facilitate service referrals once a release of information is signed
  • Maintain all participant files according to the established policies, procedures and time frames.
  • Maintain all client contact standards and enter information accordingly into GoodTrak
  • Collect and communicate all Program Evaluation and Outcomes information/statistics.
  • Demonstrate a high degree of sensitivity toward persons with disabilities/disadvantages
  • Participate on Agency committees and community organization meetings as requested





  • Prefer a Bachelor's degree in rehabilitation, psychology or related field, and 2 years of experience.
  • Working knowledge of the workforce, social services sectors and/or experience utilizing community resources is preferred.
  • Microsoft Office and Windows proficiency
  • Ability to communicate verbally and in writing
  • Must possess good time management skills with the ability to successfully juggle a variety of duties at one time, without losing composure
  • Ability to counsel and handle crisis situations effectively
  • Must possess excellent employee relations and customer service skills
  • A high degree of sensitivity towards all persons with or without disabilities/disadvantages
  • Must have reliable transportation, clean MVR, valid license and insurance to transport participants and/or drive for company-related business
